There are various methods to enlarge the breast. CenterPlast recommends the use of autologous fat if it suits the patient's wishes and conditions.
In an -own-fat breast augmentation, the surgeon first gently aspirates the fat cells using microcannulas. The cells are then cleaned, filtered and then carefully injected. A gentle surgical method ensures that the transplanted autologous fat grows and the contour of the breast is improved. The transplanted autologous fat cells also serve to increase volume. In consultation, the surgeon takes the autologous fat from the following body regions:
- Inner or outer thighs,
- Buttock region or
- The inner side of the knees.
A minimally invasive and modern method, as well as proper aftercare, ensure that the transplanted fat cells heal. The body nevertheless breaks down a certain percentage of the cells naturally. Treatment with autologous fat can achieve an increase in volume of up to 30 to 50 percent. If you are seeking breast augmentation with autologous fat by two cup sizes, you should expect to make two appointments. To achieve the desired result, the treatment can be repeated after three months. Even after breast augmentation with implants, more than one surgery is usually necessary. The limited lifespan of implants and the changes in breast tissue, make it necessary to have another procedure as you age.
- No visible scars remain.
- The breast feels more natural and softer.
- With autologous fat treatment, the breast retains a natural shape both lying down and standing up.
- Patients after autologous fat treatment recover faster than after breast augmentation with implants. At the same time, the patient benefits from liposuction in certain "problem areas".
- Any patient who chooses to have breast augmentation with implants should be aware that further surgery will be required later in life. If you want a moderate augmentation, one appointment is all that is needed for breast augmentation with autologous fat.
- With autologous fat treatment, you are spared the risks of implants: implant rupture, implant displacement, capsular contracture, more difficult breast cancer screening, breast implant-associated anaplastic large cell lymphoma (BIA-ALCL).
- After breast augmentation with implants, the U. S. Food and Drug Administration (FDA) recommends an MRI exam three years and every two years thereafter. Changes in breast tissue after breast augmentation with autologous fat can usually be easily distinguished from other diseases by experienced radiologists through ultrasound examination and/or mammography.
According to the World Health Organization, adults are underweight when their body mass index (BMI) is below 18.5 kg/m2. Many believe that breast augmentation with autologous fat is not possible because of the relatively low body fat percentage. Numerous slim women with small breasts are underweight, and many deal with the issue of breast augmentation when thinking about their own bodies. Back in 2014, a study was published by Chiu proving that breast augmentation can be performed safely on underweight women with good results. Fat cells can be taken from the abdomen, waist, hips, thighs or calves. On average, over two hundred milliliters were injected per breast. Thus, an increase in breast circumference of almost four centimeters was achieved. Almost ninety percent of the patients were satisfied or very satisfied with the result. This is also in line with our experience.
Before each procedure, the surgeon and the anesthesiologist meticulously check all parameters several times according to the checklist. In this way, all involved ensure that it takes place under the best possible circumstances. The greatest possible safety of our patients has absolute priority. Only then can the procedure begin.
Before starting the procedure, the surgeon marks the breasts and the regions of liposuction in a standing position. You will be given medication to relieve anxiety and reduce the sensation of pain. If you are not already deeply asleep, you will at most feel that something is going on at the surface of your body.
The surgeon injects the marked areas to remove the fat tissue cells ("tumescent technique"). This allows for gentle surgery and prevents the formation of bruises and irregularities in the contour. Fine microcannulas are used to inject the tissue and suction out the fat. The skin incisions of two to three millimeters are hidden in skin folds. After the tissue has been infiltrated and loosened, the surgeon removes the excess autologous fat tissue under vacuum using an aspiration cannula. The fine cannulas are inserted through several openings and guided in a fan shape to achieve an even aspiration pattern. The aspirated fat tissue cells are cleaned and filtered. The surgeon then inserts the fat tissue cells into the breast. The patient enjoys a larger breast. In addition, asymmetries or irregularities are evened out by using the body's own material. In the case of a previously sagging breast, a positive push-up effect can also be expected as a result of this method of aesthetic plastic surgery. Before the breast augmentation is completed and while you are still asleep, you will be placed in a sitting position. The surgeon will check if the shape of the breast in sitting as well as in lying position meets the highest requirements. Finally, a bandage and a special support bra are applied.
The duration of the surgery is approximately 90 minutes. The autologous fat method is performed on an outpatient basis and is also possible without general anesthesia, in twilight sleep or under local anesthesia. Immediately after the procedure, you will be continuously monitored in a so-called recovery room. After you have fully awakened, you can drink something and, if you tolerate it well, you can also eat something. Before you leave the practice, control appointments will be made and the behavioral measures will be discussed with you in detail. You will be given a comprehensive written report and a telephone number. Under this number, the surgeon can be reached 24 hours a day.
Before, after and during the procedure, you can watch movies or your favorite series with virtual reality glasses. Not only is this entertaining, the distraction has also been shown to reduce pain sensitivity.
- Your well-being and the result of the procedure will be checked regularly.
- Make sure that your upper body is elevated by 30 degrees for the first five days, even when sleeping. This ensures better decongestion and wound healing.
- You should sleep on your back for the first six weeks. This reduces the risk of the implants slipping or twisting.
- You must refrain from showering for the first few days. From the third postoperative day, you can shower before the wound check at CenterPlast Saarbrücken.
- Wear the previously fitted support bras continuously for six weeks.
- Massage the scar several times a day for a few minutes from the third postoperative week. Use an oily cream, such as Bepanthen® ointment or Linola® fat. Pay attention to circular, as well as longitudinal and transverse movements to the scar. These measures help to keep the scar inconspicuous.
- Avoid bending over, lifting heavy loads, swimming, saunas, sexual intercourse and sports activities (even with a sports bra or compression bra) for four weeks. If swelling persists, prolonged physical rest may be advisable.
Breast augmentation with autologous fat is the ideal procedure for patients who want moderate breast augmentation and a natural result. To obtain the fat tissue cells, 300 to 400 milliliters of fat tissue are needed. This corresponds to an increase of one cup size. Smaller fat pads on the abdomen, inner or outer thighs, buttocks region or the inner side of the knees are sufficient for this.
Surgery is of course always associated with certain risks. Liposuction and fat grafting can cause irregularities in the contours. However, the risk associated with this plastic-aesthetic procedure is lower than with classic liposuction, for example to remove fat deposits on the hips. The grafted fat tissue may die if the supply of nutrients and oxygen to the breast is too low. General risks (inflammation, thrombosis) and the risk of fat tissue cells not healing are increased in smokers. Fatty tissue that has not healed can liquefy and accumulate in cavities (oil cysts). In rare cases, this fluid must be removed by puncture with a hollow needle. In the case of larger oil cysts, calcium salt deposits may occur. Early detection of breast cancer is not affected by autologous fat treatment. There is no fact-based evidence that autologous fat treatment promotes the development of breast cancer. Bloodstream obstruction by the injected fat tissue is theoretically possible (fat embolism). However, there are currently no reports of fat embolisms in the context of breast augmentation with autologous fat in the world literature.

Next, your breast will be measured accurately. Even slight differences in the sides of the breast will be noted. Other characteristics that are important for planning, such as skin elasticity, any spinal curvature or chest deformities will be examined. In order to obtain sufficient fat cells, smaller fat pads on the entire body are examined.
Photographs are taken of all patients for accurate treatment planning. Based on this thorough examination and further information through a personal consultation regarding your goals, you will be shown various alternatives and all associated risks and limitations. During the in-depth consultation, the plastic surgery specialist will answer all your questions and provide you with all the information you need to decide which treatment is right for you.
