Injections with hyaluronic acid achieve excellent results in a very short time, provided the injection is actually indicated and the injection has been carried out correctly. This is the only way to achieve the desired effect. Detailed anatomical knowledge is therefore immensely important in order to achieve good results and avoid risks and side effects. Dark circles under the eyes, moderate tear troughs and slight bags under the eyes can be corrected with hyaluronic acid. An improvement in tear troughs can be achieved in around 90 percent of cases 1 . The best results can be achieved in patients with thick skin, slight skin discoloration and moderately deep tear troughs. A minimally invasive lower eyelid lift achieves the best results for wrinkles under the eyes and bags under the eyes.

Causes of dark circles

With increasing age, signs of ageing such as wrinkles and spots on the body and face become more frequent. Dark circles or dark shadows around the eyes develop to varying degrees depending on the individual. Some people experience particularly deep dark circles, giving the impression of tired eyes. Certain factors favor or accelerate the occurrence.

As the skin ages, its elasticity decreases due to the loss of collagen. The reduced moisture is usually counteracted by applying creams against dry skin. Of course, factors such as smoking, too little sleep and an unhealthy diet also have a negative effect. To prevent the appearance of dark circles, wrinkles and spots, the appearance can be slowed down or reduced to a certain extent by anti-ageing products (e.g. a hyaluronic serum), a healthy lifestyle and protection from UV radiation. However, these measures are often not sufficient or are only effective to a greater extent as a preventative measure. There may also be a genetic predisposition.

Affected people search the Internet for promising tips, home remedies and beauty products. However, only a small percentage of the hyaluronic acid in skin care products penetrates into the most superficial layer of the skin. When applied, they are not sufficient to remove particularly dark edges. This often leaves only the concealing of the affected areas of the eye. Covering the edges of the eyes with make-up is an attempt to hide the lack of volume and make chronic dark circles, which are particularly dark, disappear day after day.

One method for removing dark circles that has been proven effective in numerous studies is a dark circles treatment using hyaluronic acid injections. A natural alternative is e.g. autologous fat. Autologous fat injections offer numerous advantages. Among other things, the body’s own fat is very well absorbed by the body during autologous fat treatment. At what age should I start injecting hyaluronic acid?

This is a common question in connection with hyaluronic acid. A dark circles injection is a sensible treatment for signs of ageing on the face. Depending on lifestyle and predisposition, these changes rarely occur before the age of thirty. Hyaluronic acid is also suitable for correcting inherited facial features. Certain features such as nasal humps or a receding chin develop during the growth phase of children and can cause a great deal of suffering. After the growth spurt of puberty, these contours no longer change. Correction is possible after the first menstrual period or voice break. The filler is also suitable for treating small wrinkles or smaller wrinkles up to deep wrinkles in certain areas.

The treatment of dark circles with hyaluronic acid

Allow 20 to 30 minutes for one treatment. A local anesthetic cream is applied for your comfort. The skin is then carefully disinfected. The precise injection of hyaluronic acid is crucial for a long-term and aesthetically pleasing result. When injecting the hyaluronic acid gel, it is applied directly below the edge of the eye socket. As swelling can occur as a result of hyaluronic acid treatment, 0.2 milliliters of hyaluronic acid is sufficient for the first treatment. Nevertheless, the hyaluronic acid may be visible or palpable on very thin skin. A check-up after two weeks will show whether further treatment in the form of a second injection with hyaluronic gel is possible and necessary for your desired result. In only about 50 percent of cases is there a slight bruise after the procedure, which resolves completely after five to seven days and therefore has no effect on the patient’s appearance after these days. A slight swelling will disappear after a few weeks. A gentle massage with little pressure after the treatment helps to distribute the hyaluronic acid evenly in the tissue. In 85 percent of cases, the effect of hyaluronic acid lasts for 15 months 2 .

Filler against tear troughs

Tear troughs form at the transition between two fat pads (orbital fat and suborbicularis oculi fat: SOOF). These fat pads are separated by connective tissue fibers (orbicularis retaining ligament). The fibers of this connective tissue run from the lower edge of the eye socket through the eye ring muscle to the skin. Small changes in the skin, fat pads or connective tissue can lead to shadows forming on the surface of the skin. This can be caused by a reduction in skin elasticity, severe weight loss, water retention or signs of ageing. The following features must be considered when examining and treating tear troughs:

  • Depth of the lacrimal groove: Distance from the anterior lacrimal crest of the upper jaw (crista lacrimalis anterior) to the depth of the groove.
  • Dark circles under the eyes: Discoloration of the skin leading to dark circles under the eyes can increase the impression of depth of a tear trough. Congestion of the blood in the fatty tissue of the subcutis can also lead to darkening.
  • Bags under the eyes: If the connective tissue is weak, the central fat pad under the eye in the vicinity of the nasal bone may protrude. The prolapse of these fat pads leads to the formation of bags under the eyes and an accentuation of the depth of the tear trough.
  • Wrinkles under the eyes: Wrinkles on the lower eyelid emphasize the protrusion of the fat pad under the eye and the depth of the tear trough.

Mild forms of a tear trough are effectively corrected or removed by injecting hyaluronic acid. Most patients also experience a loss of volume on the sides, above the cheekbones. It is therefore important to include the entire eye area in the examination and treatment. In moderate or severe cases, surgical treatment (aesthetic surgery) is recommended or necessary to loosen the connective tissue and redistribute the fat deposits. Filler for upper eyelids (A-frame deformity)

An arched eyelid crease and a harmonious transition from the upper eyelid to the eyebrow lend a radiant, youthful expression. With age, the skin becomes thinner and the subcutaneous fatty tissue shrinks. As a result, the bony eye socket and the upper eyelid crease become more prominent. With a certain predisposition, a tapered eyelid crease forms on the upper eyelid, also known as “A-frame deformity”. This makes the eye socket appear larger and the eyes appear sunken. The eyelid frenulum loses its elasticity, making the eye opening smaller. This creates the impression of small eyes. The sunken area between the upper eyelid and the eye bulge can give a prematurely aged and tired impression. Sunken eyes can be corrected with hyaluronic acid injections. The effect of hyaluronic acid usually lasts for 12 months or longer.

Filler versus therapy with the patient’s own blood

The blood can be separated into its components by centrifugation. Blood platelets and messenger substances are enriched with a specific separating gel. Over 800 signaling molecules are detectable in the concentrated plasma, including vascular endothelial growth factor (VEGF), platelet-derived growth factor (PDGF) and transforming growth factor (TGF) 3 . The concentration of thrombocytes is over one million platelets per microliter and corresponds to 4 to 7 times this amount 4 . The body’s own substances are involved in many metabolic processes of tissue renewal. In the so-called vampire lift, the body’s own stimulating blood components are injected in a concentrated form. The positive properties have been known for many years 5 and proven in high-quality and meaningful studies 6 . Skin suppleness and elasticity are improved 7 . Nevertheless, certain deviations from one patient to another are unavoidable, as each person has a slightly different platelet count in the blood taken.

The concentrated blood platelets also stimulate the formation of new hyaluronic acid 8 . However, the effects are very diverse. The combination of hyaluronic acid injections and therapy with the patient’s own blood achieves better results 9 . The effects of filler and plasma complement each other. If both the skin elasticity and the subcutaneous fatty tissue are reduced, hyaluronic acid injections together with a treatment with the patient’s own blood make sense.
