What is breast augmentation with autologous fat?
During an autologous fat breast augmentation, the surgeon first gently aspirates your fat cells using microcannulas. The cells are then cleaned, filtered and carefully injected. A gentle surgical method ensures that the transplanted autologous fat grows and the contour of the breast is improved. The transplanted autologous fat cells also serve to increase volume. After consultation, the surgeon will harvest the autologous fat from the following areas of your body:
- Belly
- Inner or outer thigh,
- Po region or
- the inside of the knees.
A minimally invasive and modern method and the right aftercare ensure that the transplanted fat cells heal. Nevertheless, the body breaks down a certain proportion of the cells naturally. Treatment with autologous fat can achieve an increase in volume of up to 30 to 50 percent. If you are aiming for a breast augmentation with your own fat by two cup sizes, you will need two appointments. To achieve the desired result, the treatment can be repeated after three months. Even after breast augmentation with implants, more than one operation is usually necessary. The limited lifespan of implants and the changes in breast tissue make further surgery necessary in the course of life.
The contours of the breast can be corrected in a targeted manner with autologous fat treatment. The décolleté in particular, i.e. the transition from the breast to the neck, is given more fullness by autologous fat treatment. The proportions of the breast become more harmonious and youthful. The enlarged breast looks and feels natural.
Procedure for breast augmentation with autologous fat
Preparation
Before starting the procedure, the surgeon marks the breasts and the liposuction areas in a standing position. They are given medication that relieves their anxiety and at the same time reduces their sensitivity to pain. If you are not sleeping deeply anyway, the most you will feel is that something is happening on the surface of your body.
The OP
The surgeon injects the marked areas to remove the fat tissue cells (“tumescent technique”). Fine microcannulas are used to inject the tissue and suction out the fat. The skin incisions of two to three millimeters are hidden in skin folds. After the tissue has been infiltrated and loosened, the surgeon removes the excess autologous fatty tissue under vacuum using an aspiration cannula. The fine cannulas are inserted via several openings and guided in a fan shape to achieve a uniform suction pattern. The suctioned fat tissue cells are cleaned and filtered. The surgeon then inserts the fat tissue cells into the breast.
After the procedure
The duration of the operation is approximately 90 minutes. The autologous fat method is an outpatient procedure and can also be performed without general anesthesia, in twilight sleep or under local anesthesia.
After the procedure, you will be monitored in a separate recovery room. Once you are fully awake, you may drink something and, depending on how you feel, eat something. At the end of the surgery day, follow-up appointments are arranged.
What do I need to consider before and after a breast augmentation with autologous fat?
Please note the following recommendations for the aftercare of autologous fat treatment.
- Always sleep on your back or side, as pressure on the upper body can interfere with the healing of the transplanted cells.
- Use preformed bras without underwires. These prevent excessive pressure on the treated areas.
- Smoking should be avoided if possible. The toxins slow down wound healing and the healing of the transplanted fat cells is disrupted.
What are the risks of breast augmentation with autologous fat?
Of course, surgery is always associated with certain risks. Liposuction and fat grafting can cause irregularities in the contours. However, the risk associated with this plastic aesthetic procedure is lower than with classic liposuction, which is used to remove fat deposits from the thighs or hips, for example. The transplanted fatty tissue can die if the supply of nutrients and oxygen to the breast is too low.
General risks (inflammation, thrombosis) and the risk that the fat tissue cells will not heal are increased in smokers. Unhealed fatty tissue can liquefy and accumulate in cavities (oil cysts). In rare cases, this fluid must be removed by a puncture with a hollow needle. Larger oil cysts can lead to lime salt deposits.
The early detection of breast cancer is not affected by autologous fat treatment. There is no fact-based evidence that autologous fat treatment promotes the development of breast cancer. A blockage of the bloodstream by the injected fatty tissue is theoretically possible (fat embolism). However, there are currently no reports of fat embolisms in the context of breast augmentation with autologous fat in the world literature.

Many believe that breast augmentation with autologous fat is not possible due to the relatively low body fat percentage. A study by Chiu was published back in 2014, proving that breast augmentation in underweight women can be performed safely and with good results. The fat cells can be taken from the abdomen, waist, hips, thighs or calves. On average, over two hundred milliliters were injected per breast. An increase in breast circumference of almost four centimeters was achieved. Almost ninety percent of patients were satisfied or very satisfied with the result. This is also consistent with our experience.
Breast augmentation with autologous fat is the most ideal procedure for patients who want a moderate breast augmentation and a natural result. 300 to 400 ml of adipose tissue is required to obtain the adipose tissue cells. This corresponds to an increase of one cup size. Small fat deposits on the stomach, inner or outer thighs, buttocks or inner knees are sufficient for this.
